NTSB Blames Pilot Error and Poor Weather in Fatal Crash That Killed Herbert Wigwe and Others

The U.S. National Transportation Safety Board (NTSB) has released its final findings on the tragic helicopter crash that killed former Access Holdings CEO Herbert Wigwe, his wife Doreen, their son Chizi, and ex-NGX chairman Abimbola Ogunbanjo on February 9, 2024.

The investigation concluded that the pilot's decision to fly visually in deteriorating weather conditions led to spatial disorientation and ultimately, loss of control of the aircraft. 

The crash occurred near the California-Nevada border, claiming all six lives on board.

In addition to pilot error, the NTSB cited the helicopter company's "inadequate oversight" as a contributing factor. The operator failed to enforce critical safety measures, including pre-flight risk assessments and proper maintenance documentation
